Some plans do not cover immunizations, hearing tests, vision tests and lab work. Please go on-line with your insurance to verify your coverage and that we are a provider within your insurance network....BEFORE your appointment!
Please have your card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of visit. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.

Your well visit is intended to be an exam of your health, your health history and an assessment of what you may need for health screenings, risk management, vaccinations and other preventative care.
Adolescent sports physicals are well, preventative visits. A wellness, preventative visit is different from the visits where our providers diagnose or treat specific problems. We understand that sometimes it is necessary to diagnose new or specific problems while you are here for your wellness exam. We want to make our patients aware that in order to comply with insurance mandates, we must separate charges for wellness and problem based exams.
Depending on your insurance, this may result in a copay being charged or a visit being applied to your deductible. Medical billing is often confusing so please never hesitate to call and speak to our billing staff if you have any questions or concerns regarding your statement.
